Model Photographers

Where on the internet do I find sample limited model/photographer/agency release form templates?
I am looking for online model/photographer/agency limited release forms (templates) that are on behalf of the protection of the model.
More specifically, I am looking to find a template that looks out for the privacy of the model such as in the case of selling/blanketing images without the models permisson.
Actually “model” releases protect the photographer and model equally or they are not enforceable
